Oakville Kiwanis Meals on Wheels Referrals Referrals for service come from many sources. The person interested in receiving meals may call themselves to request service. Family, friends, physicians, social workers, Community Care Access Centre and others interested in the care of an individual may call and refer someone to the program. A brief interview is conducted over the telephone to obtain the required information. This information is very basic and includes information such as name, address, telephone, date of birth, reason for service, diet information, next of kin or contact person, name of physician and other related details. It is our policy, in the interest of safety, to follow up on clients when there is no response at the time of delivery. |
